Registered Nurse Case Manager
Provide teaching and management on diseases; wound care including wound vac; medication management; ostomy care/education; IV therapies including medication, nutrition, fluids; and other services as prescribed.
Physical Therapy
Helps in improving Range of Motion, mobility, endurance, gait, balance, strength, posture, safety awareness, fall prevention, pain issues and transfers due to physical impairments.
Occupational Therapist
Works to improve Activities of Daily Living including bathing, dressing, grooming, and transfer ability. Also helps in providing adaptive equipment training and low-vision strategies.
Speech Therapy
Provide training, techniques to promote effective swallowing, cognition training, and speech techniques.
Medical Social Worker
Assist in locating community resources available to support patients and families during recovery.
Certified Home Health Aide
Assist after acute illness or injury with Activities of Daily Living until patient and family are able to perform these tasks independently.
Dietician
Determine nutrition risk of home health clients, and develop an individualized plan of care for their diet.
